Nicole Gullick, M.D.
Transformations by M.D. was founded by Dr. Nicole
Gullick, who has dedicated her life to learning about
health, nutrition and fitness. She obtained her
medical degree from the University of Southern
California. She has lived in Arizona since 1994
and has worked in many different areas of medicine,
including primary care, urgent care, emergency medicine,
hospital medicine, intensive care, and physical
rehabilitation. In caring for patients at all stages of
disease, from initial diagnosis to end-of life, she
quickly recognized that there is an
undeniable deficiency in true preventive
health. She was struck by the realization that
medicine has become a model of "disease care",
not healthcare. The continued rise in cancer
and heart disease along with our overcrowded hospitals
and emergency rooms are just a few indications that the
current model is not working. She became
determined to transform the face of healthcare
by helping people take charge of their own
health.
Lisa Clyde,
Aesthetician
 Lisa
is a licensed aesthetician who is trained in all phases
of assessment and treatment of blemished skin. She is
certified to perform microdermabrasion, chemical peels,
photo facials, and laser hair removal. She gained her
valuable experience by working for a plastic surgeon as
well as a dermatologist for several years prior to
starting her own business in skin rejuvenation. She has
a very loyal clientele due to her extensive knowledge,
experience and ability to recommend appropriate product
lines and perform procedures for accomplishing
rejuvenation of the most challenging skin textures. We
are very fortunate to have her as part of our team to
help our clients transform their appearance and boost
their self esteem. She will literally peel away any
discontent you may have with your complexion and undo
the effects of aging, one layer at a time.
Katie O’Brien, PA-C, MMS
Katie O’Brien graduated with a Master’s
Degree in Physician Assistant Studies in 2001 from
Saint Louis University. After graduating, she
practiced as a PA in Neurosurgery and Orthopedics but
then joined the field of Dermatology where she has
functioned as both a medical and cosmetic PA since
2003. She recently relocated to Tucson in July of 2009
from Branson Missouri, where she both established and
operated the area’s first and only Medical Spa.
Additionally, Katie formed her own national aesthetic
injectable training company that instructed other
health care professionals in the art of Botox, Dermal
Fillers and LipoDissolve. Her extensive knowledge of
nonsurgical aesthetic procedures, including laser
treatments, as well as her background in Dermatology
make her an invaluable member of the Transformations
by M.D. team.
 Barbara
Cravatzo, Permanent Make-up Artist
Barbara is a Tucson native who began her career
in cosmetology as an electrologist. She
eventually received training in cosmetic tattooing and
has been performing this art for over 20 years.
With her keen eye for facial aesthetics and her
skilled hands, she has enhanced the beauty and
balanced asymmetry in countless men and women.
She produces a look that is natural and endures the
test of time.
